Beyond the Chart

This is a 16-hour online course. 

Every patient care report tells a story, but the most important details often lie beyond the documentation itself. High-performing EMS agencies use Quality Assurance (QA) and Quality Improvement (QI) not to find fault, but to uncover opportunities that improve patient care, provider performance, and system reliability.

Beyond the Chart is an immersive four-week course that places students in the role of a newly appointed Quality Improvement Coordinator for a fictional EMS agency. Through realistic patient care reports, CAD records, monitor data, operational metrics, patient complaints, and quality dashboards, participants learn to investigate clinical performance, identify system trends, and transform findings into meaningful improvements.

Rather than simply teaching how to review charts, this course teaches students how to build an effective EMS quality program from the ground up. By the end of the course, learners will have developed a complete Quality Assurance framework, including chart review criteria, performance indicators, review workflows, and a Quality Improvement initiative, that can be adapted for implementation within their own agency.

This course is designed for EMTs, Advanced EMTs, Paramedics, Field Training Officers, Clinical Coordinators, EMS Educators, Supervisors, and current or aspiring EMS leaders seeking practical skills in quality management and organizational improvement.

Learning Outcomes

Upon successful completion of this course, students will be able to:

• Explain the role of QA/QI in modern EMS systems.
• Conduct structured reviews of EMS patient care reports.
• Evaluate documentation for clinical, operational, and legal quality.
• Apply Just Culture principles during chart review.
• Identify individual performance issues versus system-related issues.
• Analyze agency quality metrics and identify trends.
• Design meaningful quality indicators for EMS agencies.
• Develop an agency-specific chart review process.
• Create an EMS Quality Improvement project using the Plan-Do-Study-Act (PDSA) model.
• Build a comprehensive Quality Assurance program suitable for implementation within an EMS agency.
